What language do you need to translate into? If in English / German / Dutch, then in any case it will most likely be like Fyodorovna (at least from Russian, at least from Ukrainian). Then, I don’t know how it is in Ukraine, but in Russia, if you don’t have a foreign language diploma (translator), then a notary will not certify such a translation for you. And if they translate for you in a special agency, then they themselves know how it is done.

No, from Ukrainian it will be "Fedorivna". I usually translate according to the source document. Notarization is not required.

As a matter of fact, you also made a mistake twice - about transcription from the Ukrainian language and about a notary. Why even write something if you don’t know or are not sure? I don’t know how it is in Russia, but in Ukraine it’s not accepted :)
