Documents required to obtain a single-entry visa for a stay of up to 90 days:
AND COMPLETED APPLICATION FORM - visa application,
II CURRENT TRAVEL DOCUMENT OF THE APPLICANT (passport) + copy of pages with data. Please note that the passport must:
a) be valid for at least three months after the return of the person to the territory of Ukraine,
b) have at least two free pages (pages for special marks are not taken into account),
c) be issued within the last ten years;
III CURRENT CIVIL PASSPORT OF THE APPLICANT + copy of 1, 2 pages and registration of residence,
IV 1 COLOR PHOTOGRAPHY (3.5 x 4.5 cm) - 80% of the photo should occupy the face,
V ORIGINAL INVITATION ON THE OFFICIAL FORM, managed by the local body of the Police Service for Foreigners (Foreign Police) + 1 copy of this document
