From my own experience I will say that it is possible to enter in the passport (one of the parents) within 1 week-10 days, and the travel document can take longer. Moreover, they write it in the passport and paste the photo in the passport office, and not in the avira (at least in Sumy - this is true, but, I think, this is all over Ukraine)

If the photo is pasted into the passport of one of the parents, then if the mother and father are traveling, nothing else is needed. And if only one of the parents, then a notarized permission of the second (notaries know the established form) is needed for consent to take the child abroad. the same thing, if there is only one parent, you also need a notarized document.
