From what we have personally seen on Koh Samui, we can recommend the Samui Orchid Resort hotel as for a child. Shallow sea, silence, zoo, performances with animals. As for civilization - either by taxi or minibus 15-20 minutes to Lamai. It is easier to take a jeep for the entire period of rest.

Then another by car for about 4 hours, by ferry, again by car and in place. Had a great rest. Now he is almost 8, he has been to Spain twice and to all the sights, he has been to Sri Lanka twice, including in Kandy and on a safari. This year we are planning Africa Tanzania safari. Do what works for you and everything will work out! And yes! Thailand is a very civilized old, with developed medicine (definitely better than ours).
