I don’t know how exactly the sewerage system is arranged in hotels on the Kvai River, but the water is much cleaner than in the glorious Dnieper River. Most likely, wastewater accumulates in tanks and is then transported by road. Who scared you so much? Do you really think that the Thais are so "wild" - to directly dump waste into the river?

It so happened that I was on this excursion twice in different years, the water in the river is cleaner than in some of ours, I only met a coconut that fell into the water, which tourists immediately used instead of a ball, so they rafted fun.)) In my opinion, this is one of the most interesting excursions, I liked the option where both the Erawan waterfall and thermal springs are included, so take it you will not regret it.
