I am from the Lugansk region, and she is from western Ukraine, and she will easily issue a shopping visa, but it’s not realistic for me. I do not know, unfortunately, how things are with Polish visas. Our nearest consulate is in Vinnitsa, there are no problems with visas. The application form (questionnaire) can be downloaded from the website of the consulate. And look for some Polish organization in your city - usually there, for little money, they provide assistance in filling out a questionnaire and preparing a package of documents. Through the Internet, you can also register for the submission of docks to the consulate.
As far as I know, you will need: a passport (valid for at least 3 months on the day you return to Ukraine), a blue ink application form (application form), 2 color photographs (in the photo studio you will say that for a Schengen visa), a copy of all pages of the internal passports on separate sheets of A-4 format, a copy of the identification code, a copy of the birth certificate, copies of the first page of the passport and all pages with marks - on separate sheets of A-4 format.
